Reflection Paper About the Movie “Every Child Is Special” Essay

REFLECTION PAPER FOR THE MOVIE â€Å"EVERY CHILD IS SPECIAL† Ishaan Awasthi an eight-year old whose world is filled with wonders that no one else seems to appreciate and who were always getting in trouble at school for being so misbehaving and out of focus from his lessons. Too often, he would be caught by his teacher daydreaming and getting low grades. He even cannot read nor write, for him letters and numbers are his enemy. For the people around him, Ishaan is a naughty and lazy boy because of this, his father sent him to boarding school, all alone and homesick with the hope of disciplining him, but the academic status of Ishaan did not still improve. Instead, he became withdrawn and lonely, far from the Ishaan who was active and fun-loving. Then came a new art teacher Ram Shankar Nikumbh who infects the students with joy and optimism. He breaks all the rules of â€Å"how things are done† by asking them to think, dream and imagine, and all the children respond with enthusiasm, all except Ishaan. But with Ram Shankar Nikumbh’s time, patience, care, and some awesome motivation he ultimately helps Ishaan find himself and change the way Ishaan would act towards school and learn to appreciate himself even more. Ishaan Awasthi is not an ordinary student. He has a very wild and creative imagination that brings him to different worlds, to worlds unknown to his classmates, teachers and family. He usually plays and does things on his own, he has his own special perception on things. Though his talent in painting and creativity is exceptional, he is just like every other kid, he loves to play and goof around. But the problem was, he cannot not cope with school the way other kids does. He has repeated the third grade, and still he did not show any improvement. He fails to read and write properly that’s why he was unable to pass his subjects. At the same time, he cannot attend to himself; buttoning his shirt or tying his shoe lace. A kid at the age of 9 is capable of all th ose things, unfortunately for Ishaan he is not, because he suffered from dyslexia. The teacher-student relationship here is just so heartwarming. In fact, it’s more touching than Ishaan’s relationship with his parents. On his first day of school, Ishaan’s teachers were upset by his poor performance in class. His Art;s teacher even whacked his knuckles five times. Ishaan was overwhelming and traumatized at the same time because of  how strict the teacher. He became depressed and sad. There is also a scene where Ishaan runs away from school and loiters on the road for the whole day – for the fear of being punished for not having done his homework. I never did anything like that in school. The comparison might not be warranted but this is just one of those scenes which made me relate to Ishaan’s plight in the movie. The incidents involving the teacher-student interactions depicted in this movie are so relevant in today’s times when you get to see and hear on television at regular intervals innovative punishments like whipping and punching done to students. He once complained that â€Å"The letters are dancing!† when he was asked to read. Teacher threw him out of the class and the student who passes by the hall mocked him for being punished. Moreover, Ishaan rev ersed letters when he wrote and demonstrated a poor understanding of mathematical concepts. Sometimes if he commits mistakes everybody laughs at him or will shout on him. A scenario that was related to me during my elementary year that was related to Ishaan, is being bullied and put to shame by my teacher. Everyone is challenged by obstacles in his or her life – serious problems to deal with. Some people are defeated by the obstacles in their lives, while others find way to overcome them. I believe that everyone has problems or challenges to overcome. Even people who think that they are perfect have problems or challenges to overcome. Life is beautiful but not always easy, it has problems, too, and the challenge lies in facing them with courage, letting the beauty of life act like a balm, which makes the pain bearable, during trying times, by providing hope. As a student, who burns mid night oil, makes sacrifice and resists temptations so that I can perform well. Similarly, a successful executive has to face the ups and downs of life, not forgetting the life is a mix of success and failure, joy and sorrow. I overcome my problem by means of self-steam and through the help of other people I meet in this world. Ishaan overcome his problem through the help of Ram Shankar Nikumbh. Difficulties test the courage, patience, perseverance and true character of a human being. Adversity and hardships make a person strong and ready to face the challenge of life with equanimity. There is no doubt that there can be no gain without pain. It is only when one toils and sweats it out that success is nourished and sustained. Thus, life is and should not be just a bed of roses; thorns are also a part of it and should be accepted by us just as we accept the  beautiful side of life. And those, who think that good times last forever, succumb to pressure during difficulties. They do not put in required hard work and efforts because they break down easily.

Application Case – The Out-of-Control Interview 1. How would you explain the nature of the panel interview Maria had to endure? Specifically do you think it reflected a well-thought-out interviewing strategy on the part of the firm or carelessness on the part of the firm’s management? If it was carelessness, what would you do to improve the interview process at Apex Environmental? It is possible that the panel interview of Maria Fernandez at Apex International was a stress interview designed to determine how she reacts under pressure and with a barrage of irrelevant and sexist questions. However, stress interviews are viewed as unethical and of poor taste. Apparently, Apex International management was careless in its interview strategy. First, it constituted a panel of five men who maybe well acquainted with the company’s operations but could be possibly lacking in experience in HR functions, particularly in recruitment and selection. It is assumed that the HR manager or any HR specialist who could have provided structure, direction and control to the interview was not part of the panel. To address this concern, it is highly recommended that Apex International conduct trainings for interviewers in order to maximize utility of interviews as a tool at arriving at a selection decision as well as to avoid straying off course and asking questions that have potential job discrimination implications. 2. Would you take the job if you were Maria? If you’re not sure, is there any additional information that would help you make your decision, and if so, what is it? I am unsure in taking the job offer. I would take into careful consideration the company’s Code of Conduct and Ethics. Based on what happened, it appears that there are concerns on the culture of the company, particularly on sexual harassment and equality in the workplace. Questions asked during the panel interview indicate loose policies on sexual harassment. While interviews are primarily designed to find out if the job applicant has the experience and skills needed for the position, it could also be a tool for the company to emphasize its culture, leadership, among others. . The job of applications engineer for which Maria was applying requires: (a) excellent technical skills with respect to mechanical engineering; (b) commitment to working in the area of pollution control; (c) the ability to deal well and confidently with customers who have engineering problems; (d) willingness to travel worldwide; and, (e) a very intelligent and well-balanced personality. In the film, â€Å"Lion King† King Mustafa refers to the valley as the â€Å"Circle of Life† which is significant for the cause of preservation of animal life. The incidents that occur later in the story when Scar lets the hyenas loose on pride land demonstrates the disastrous results when animals are used for selfish purposes and when their lives are not preserved. This is an important message that only serves to emphasize how important it is that animals should be made to suffer, neither should they be used indiscriminately for man’s benefit, as in the entertainment industry and in scientific experiments – where they are put through pain and suffering. Killing animals is also likely to disturb the Circle of life and the delicate ecological balance that exists. This is the real problem the film addresses with its underlying message - the cruel and indiscriminate killing of animals. The film applies subtle persuasion as far as the viewer is concerned, highlighting the importance of preservation of the life of animals in order to ensure that the various species of the earth are protected. The text of the film supports the message against indiscriminate killing of animals, mostly through the stark desolation it portrays of the land when most of the animals in it have been destroyed. This is an important message in the modern day context, when man is indiscriminately destroying animals and subjecting them to cruel and inhuman treatment. As the film portrays, there is a delicate balance in nature, where different species co-exist harmoniously together. Pride Land as it existed was a place where there was a productive earth and happy animals because none was subjected to cruel degrading treatment. The film also provides a resolution for the problem. The Amadou Diallo case study was an example of the tragic error which was made by four New York City Police officers. The police shooting of an unarmed man was an act of automatic inferences which happens when people use mental shortcuts to simplify the amount of information they receive from the environment. Automatic thinking is known as the thinking that is unconscious, unintentional, involuntary and effortless (Taylor, Peplau, Sears, 2006). While, schemas are mental structure people use to arrange their information regarding the social world around themes or subjects: schemas affect what information we notice, think about, and remember (Chapter Review, 2010). During the incident February 4, 1999, Carroll had made a low-effort automatic thinking with schemas when Diallo reached into his jacket to get his wallet by assuming that Diallo was reaching for a gun in his pocket, and shouted â€Å"Gun! † to alert his colleagues. Officer Carroll’s action was due to his natural instinct or response as most criminal would reach into their pocket for gun during the detection of police officers. He had use schemas to form an expectation of the event in which made him to expect a gun pulling out of Diallo’s jacket rather than his wallet. As he attends to his schema-consistent knowledge: criminals would pull out guns from their jacket when they spotted police officers; his schemas filtered out any inconsistent information: Diallo reached in his jacket to get his wallet; had caused the NYPD officers to fire at Diallo. Perseverance effect is known as the tendency for people’s beliefs about themselves and their world to persist even when those beliefs are discredited (Taylor, Peplau, Sears, 2006). When the four NYDP officers thought Diallo matched the description of a (since-captured) serial rapist, it might be due to the fact that Amadou Diallo was an immigrant to the United States and the four police officers could have the belief that immigrants are the major causal crime increment in the Unites States. Hence resulting the police officers to think the worse out of Diallo when he ran and even though he was reaching into his jacket to get his wallet, the police officers had persisted that the square object had been of a firearm although in logical terms both wallet and gun do not share the same shape and size. This might be because that Diallo was a dark skin immigrant which had logically matched the description of the serial rapist leading officers to made a quick decision so that they do not need analyse in detailed and make a major hold-up over the matter. Furthermore, the four police officers also did performed availability heuristic in this case. Availability heuristic is a mental rule of thumb whereby people base a judgment on the ease with which they can bring something to their mind (Taylor, Peplau, Sears, 2006). During the incident, Diallo had been mistaken that he was reaching into his jacket to get his firearm whereas he was actually reaching to get his wallet. Officer Carroll had shouted â€Å"Gun! † to his colleagues. His judgment might be due to his ease with which he can bring to his mind, which in this case, was automatically thinking that Diallo’s wallet was a gun, hence warned his colleagues about the matter. Attitude heuristic is the determination of what is â€Å"true† based upon an individual’s feelings towards or for a matter (Taylor, Peplau, Sears, 2006). The four police officers also had carried out attitude heuristic towards Amadou Diallo. For example, these police officers might dislike Diallo because he was an immigrant or ‘black’ person therefore assuming that Diallo’s intention, plan, or doings would also be bad too. This caused Diallo to be misinterpreted to be a criminal trying to escape from police detention rather than his true intention, which was believed to initially intend to show the NYPD officers his identification card in his wallet before he wrongly shot. Besides theories and concept of automatic thinking with schemas, the Amadou Diallo case had also displayed a few relevant theories of social perception. During the incident, the four police officers had displayed the attribution theory. Attribution theory is a description of the way which people explain the causes of their own and other people’s behavior (Taylor, Peplau, Sears, 2006). It is because that Diallo had run up the outside steps of his apartment after they claimed to identified themselves as NYPD officers, and that he had reached into his jacket, the police officers have made a cause and effect explanation towards Diallo’s behavior hence assuming that he was a criminal or the serial rapist they were searching for, therefore Diallo had run up in the attempt to escape and when he reached into his jacket for his wallet, they thought that he was reaching for his gun to defend himself. Due to this theory, it made sense of he reason four police officers have assumed that Diallo was a criminal and they had to fire back Diallo. All this might be due to the attachment of meaning they decode from Diallo’s nonverbal behavior. In addition, the fundamental attribution error was also shown in the incident when the four police officers have focus on Diallo’s action to run and reached into his jacket, rather than the situation causes of his action, which was intending to reached into his jacket to get his wallet to show the police officers that he was innocent when he was approached by the police officers and asked to â€Å"show his hands†. Fundamental attribution error is known as the tendency to overestimate the extent to which a person’s behavior is due to internal, dispositional factors and to underestimate the role of situational factors, one reason people make fundamental attribution error is because the observer only put their attention on the actor, while ignoring the situational causes of the actor’s behavior (Taylor, Peplau, Sears, 2006). Furthermore, Diallo had also displayed the spotlight effect in this case study. The spotlight effect is the tendency to overestimate the extent to which our actions and appearance are salient or clear to others (Taylor, Peplau, Sears, 2006). When Diallo had reached into his jacket to get his wallet, he might had assumed that the NYPD officers know or understand what he was doing, which in his case, was the probability to take out his wallet and show them he was not the criminal or man they were searching for. However, the four police officers were not aware of his action hence assumed that he has a gun, hence fired at Diallo. Moreover, police officers had also displayed the aggression objects as cues concept during the shooting event. Aggression objects as cue is an aggressive stimulus that act as an object that is associated with aggressive response, and whose mere presence can increase the probability of aggression (Taylor, Peplau, Sears, 2006). The aggressive stimulus in this Diallo case is the guns that the police officers possessed. When the police officers had encountered a threatening or violent stimulus, they were more likely to â€Å"shoot† regardless of what stimulus actually appeared (Baumann Desteno, 2010). Due to the possession of firearms, the four police officers had the higher possibilities to response aggressively towards Diallo, hence the gun act as the object that associated with their aggressive response. Without the firearms that the four police officers possessed, the act of aggression towards Diallo would decrease therefore the firestorm would not have happened. According to Correll et al. (2002), it was found that participants’ performance shown difference regarding race of the targeted person. When a decision making process was time limited, participants had made more errors shooting unarmed African American targets than unarmed White targets. Whereas, participants had made more errors not shooting armed White targets than armed African American targets. In other words, participants were more likely made errors evaluating African American targets as threatening compared to White targets. This means that because Diallo was a ‘black’ immigrant, it increases his rate of being shot. This is an act of stereotype as because Diallo was a ‘black’ immigrant, they assumed that he might be a criminal, hence the police officers thereby increasing the likelihoods for certain interpretations and ensuing actions during the crucial moment, resulting them to mistaken Diallo’s wallet as a gun, hence shot him. In another words, it is like the Arabs possess guns, resulting in more frequent mistaken â€Å"shootings† of Arab men within the context of the shooter bias paradigm (Unkelbach, Forgas, Denson, 2008). There are also group prejudices that had been displayed in the Amadou Diallo case study. The four police officers had shown prejudice against Diallo because he was a dark-skin immigrant, hence they have ordered him to stop and â€Å"show his hands† without confirming whether he was the serial rapist they were searching for. Seeing the suspect holding a small square object, Officer Carroll yelled Gun! to alert his colleagues, believing that Diallo had aimed a gun at them at close range, the officers opened fire on Diallo. Diallo was a West African immigrant with no criminal record (Cooper, 1999). It was clearly shown the act of prejudice towards Diallo, whom was misunderstood or misinterpreted just because of his race. Furthermore, discrimination was shown in the Amadou Diallo case when the four NYPD officers had opened fire to shoot Diallo without proper investigation on his identity and the square object that he was holding during the incident. The action carried out by the four police officers was unjustified and harmful towards Diallo because he was a West American immigrant in the United States. Finally, illusory correlation was also shown in the Amadou Diallo case. An illusory correlation is the tendency to see relationships, or correlations, between events that are actually unrelated (Taylor, Peplau, Sears, 2006). When Diallo ran up the outside steps of his apartment and pulled out his wallet, the police officers had correlated his action to a criminal’s doing hence leading them to assume that he was a criminal ready to open fire at them in short range. These two events are distinctive and hardly correlated. Because Diallo was a West African immigrant, therefore the police officers correlated the two events together, causing Diallo his innocent life.

The Good Life Essay What is the good life? This ambiguous idea differs for everyone. Americans may believe that to live the good life is to live the American Dream and achieve freedom, equality, and prosperity. Others may believe that the good life involves being very wealthy or famous. Over all, the definition of the good life varies for everyone. There isn’t one specific way that a person should live in order for it to be â€Å"good†. However, although the definition of the good life is very diverse and open to multiple interpretations, in order to live the best life possible, one should concentrate on doing whatever makes him or her happy, listening to his or her conscience and doing what it right, and establishing good relationships with family and peers. The most key piece to living the good life is doing whatever makes one happy. For most adults, this could mean having a job in their dream career field, and actually wanting to get out of bed to go to work in the mornings. Or, it could mean not having a job at all. Nonetheless, most adults would be happy by achieving success in their chosen careers, and having stable and loving families. However, a small number of adults actually achieve this ideal happiness. Regardless, the good life is still attainable. In order to live the good life, one has to strive to find happiness in whatever he or she does. According to Ralph Waldo Emerson in his essay â€Å"The Over-Soul†, one can achieve happiness through God and religion. He states that God is accessible to everyone, religious or not, and that religion is essentially the key to all happiness. For Christians, Emerson’s point of view may be correct. Many Christians believe that God can bring happiness to anyone who believes in Him. For non-Christians on the other hand, Emerson’s idea is irrelevant. Still, religious or not, people should strive to find happiness in whatever they do, and they should learn to accept their lives as they are. Even though no one lives a perfect life, people should aim to make the most out of their lives, and learn to find happiness in even the smallest situations. Doing so would allow one to become closer to attaining the good life. Another important part to living the best life possible is listening to one’s conscience and doing the right thing, and actually feeling good about the decisions that are made. If one’s life consists of doing bad things, he or she will ultimately feel bad about them, and many regrets will be left behind. However, if one does a lot of good deeds throughout his or her life, there will be minimal regrets and he or she will be able to work towards living the best life possible. Listening to one’s conscience is also an effective way of avoiding regrets. Many times, when one doesn’t listen to his or her conscience, regrets occur, because one looks back on the decisions made and realizes that they should have done something differently or listened to their conscience. Even though listening to one’s conscience might not always be the right thing to do or yield the best results, it will allow for one to live with few regrets. And, by living with few regrets, doing what is right, and listening to the conscience, one will become closer to achieving their good life, in whatever way it is defined. The last way to live the best life possible is through establishing good and meaningful with one’s family and peers. A lack of friendships and substantial relationships will most likely leave one feeling very lonely and outcast. However, if one can form positive relationships, they will be more social, happy, and closer to achieving the good life. In Gary Eberle’s text, The Geography of Nowhere, Eberle states that electronics and the media are preventing many people from establishing good, intimate relationships with others. Electronics are causing us to be very disconnected from each other: â€Å"The people spend so much of their lives connected electronically to someplace else that most of the time they hardly seem to be there, or anywhere at all, for that matter† (Eberle 2). Here, Eberle is showing how electronics and the media have caused society to be very disjoint, and they have hindered meaningful relationships from forming. So, in order to live the best life possible, we should strive to overcome the power of electronics and try to continue to make successful relationships with others, as healthy friendships are one of the most important pieces to living a good life. Still, working past the power of electronics is not the only way to secure good relationships. In addition, one ought to be a trustworthy person. Trust is essential to all relationships, and if one is trustworthy, he or she will have more satisfying relationships with others. Along with trust, in order to form good relationships, one could be understanding, generous, comforting, and more. Doing so will allow one to develop healthy relationships with their friends and family, and the good life will then be more reachable. Over all, the good life remains a very unclear idea, and there is no concrete definition on what the good life is, nor is there a definite way on how one is supposed to live it. Still, one can work towards living the best life possible by striving to find happiness in his or her life, as well as listening to his or her conscience, doing what is right, and establishing valuable relationships with peers. If one can do all of these things, they will be closer to achieving their definition of a good life.

Episiotomy Rates Vary Widely Worldwide Labour is a wondrous act of nature and unique to every childbearing women. It is a transformative and special event in a womens life. It is the magic of creation. The very process of giving birth is the most beautiful one on earth and the mother attains unique capacities and true nobility through childbirth. The onset of motherhood present a unique set of physical, emotional and psychological challenges. Episiotomy is a common surgical procedure performed during second stage of labour. The first performance of episiotomy was done in 1742, when perineal incision was made to facilitate difficult deliveries (Grass, Dunn and stys 1986). It is made both to prevent tearing of the perineum and to release pressure on the fetal head with birth (Lawsan and Bienstock, 2007). It is the only procedure in obstetrics is performed without the patients specific consent. The advantage of an episiotomy is that it substitutes a clean cut for a ragged tear, minimize pressure on the fetal head, and may shorten the last portion of the second stage of labour (Incerpi,2007). Episiotomy rates vary widely worldwide, depending on whether the procedure is used restricvely / routinely. The worldwide episiotomy rate was 27%, 54% are nulliparous and 6% are multiparous women (WHO 2003).Rates vary from 8% in the Netherlands,13% in England to 25% in the USA. Among English speaking countries, the US had the highest episiotomy rate, varying greatly from region to region. One in three mothers who delivered vaginally in the U.S from 1995 to 2003 had episiotomies. In India the birth rate is very high 56% of women had an episiotomy compared to the 46% of white women. The difference between these percentage (10%) is measure of the excess frequency of episiotomy in Indian women. According to the American college of Obstetrics and Gyenaecology, approximately one in three women having a vaginal delivery also have an episiotomy. Approximately 70% of women with a vaginal birth experienced some degree of damage to the perineum due to tear (or) episiotomy and needed stitches. Like any other surgical incision, episiotomy results in some discomforts for most of postpartum patients (Hill,2000). Although relatively small in size, episiotomy sutures can cause considerable discomfort, because the perineum is an extremely tender area and the muscles of the perineum are involved in so many activities such as sitting, squatting ,bending ,urinating and defecating. Even without episiotomy women may experience bruising / tearing in the perineum. This is the most common source of infection in the days after giving birth. Various interventions are found to reduce episiotomy wound infection and enhance the healing process which include administration of antibiotics, cleanliness, topical application of ointments, infra red therapy, sitz bath, performance of kegals exercise and perineal care (Helen et.al 2009). In addition complementary therapy is another great aid to perineal healing. In recent years it has been seen that there has been a shift people from allopathy medicine to complementary medicine. The shift may be because people are becoming more conscious of their health and realize the need of holistic health. Quality of life is felt more important. In 2007 National health interview survey of complementary medicine use by Americans, showed that approximately 38% adults use complementary medicine. A survey by NPS medicine wise in 2008 revealed that 65% of Australians had used one (or) more complementary medicine in the previous 12 months. The term complementary medicine is primarily used to describe practices employed in conjunction with (or) to complement conventional medical treatment. Complementary medicine is sometimes called mind body medicine because it is an approach to healing that uses the power of thought and emotions positively influence physical health ( USA Drug, 2005). Complementary therapies can help to boost the immune system, help eliminate toxins, help relieve pain, improve sleep pattern, increase energy levels, induce sleep relaxation, reduce stress and tension, restore balance to body systems. Lavender oil is one of the complementary medicine. It has a light fresh aroma, is clear in color and watery in viscosity. Lavender has been use continuously for thousands of years, either in the form with lavender water of essential oil / dried flowers. In the 1800s the yardly company in England, realizing the healing properties of lavender , began adding it to their soaps and perfumes. Lavender oil promotes rapid healing and helps to prevent scarring. These remarkable healing properties of lavender were accidently discovered by French chemist Dr. Rene Maurice Gattefosse when he burnt his hand in a laboratory experiment. Lavender oil is found as a good choice in treating episiotomy wound healing because of its cell regenerating properties. Need for the Study The postnatal period refers to 6 weeks period after childbirth. The period is popularly termed the fourth trimester of pregnancy. It comprises an amazing variety of complex physiologic and psychological adaptations. Protecting a womenà ¡Ã‚ ¾Ã‚ ½s health as these changes occur is important for preserving the future childbearing function and for ensuring that she is physically well enough to incorporate her new child into her family. The physical care a women receives during the postnatal period can influence her health for the rest of her life. The nurses role is vital as she assists the mothers through these adjustments and supports them as they make a fresh start as a new family. Postnatal women are more prone for puerperal infection as a result of episiotomy which can be prevented by proper postnatal care. Approximately 33% of women with vaginal delivery had episiotomy in 2000 (American College of Obstetricians Gynaecologist, 2008). However the prevalence of episiotomy is not the same in different countries, Asian race are presumed to have smaller and tighter perineum so, the routine episiotomy may reduce the risk of Perineal tearing during delivery (lam et.al 2008). Studies about the episiotomy rates around the world showed that this surgery ranged from 9.7% (western Europe-Sweden) to 96.2% (South Africa-Ecuador) with lowest episiotomy rates in English speaking countries (North America Canada united states) and it remained high in many countries (centered south -America like brazil-94.2%, south Africa-63.3% and Asia like China-82%). One study revealed that episiotomy was performed in 97.3% of 510 primiparous women who had vaginal delivery in Tehran (Shojariet.al.2009). Barnabas (July 2012) conducted a study to determine the rate and risk factors for episiotomy at Baptist medical centre, Nigeria. Out of 280 subjects who had vaginal delivery, 96 (34.3%) subjects had episiotomy .The rate of episiotomy decreased with parity. The nulliparous had the highest rate (62.2%). The episiotomy rate among assisted vaginal delivery was 80.0% among those who had spontaneous delivery. Law kw, wong Hs, Pan Tc (2006) were reviewed the use of episiotomy during vaginal delivery in Hongkong puplic hospitals. A prospective observational survey was carried out in total 6222. Women who underwent normal vaginal delivery of a singleton fetus with cephalic presentation of the 6167 women in whom the status of the perineum was known. Episiotomy was performed in 5274 (85.5%). Primiparous women were more likely to undergo episiotomy at delivery than multiparous women (97.9% vs. 71.4%). Several studies suggest that using episiotomy in normal delivery results in fewer perineal laceration and trauma. The episiotomy discomfort and its consequences can affect maternal quality of life as well as the mothers and baby relationship. Like any other surgical procedure episiotomy carries number of risks: excessive blood loss, hematoma formation and infection ( Roberts and Chalmers 1989) S.T Sule and S.D. Shittu ( July 2003) conducted a prospective cohort study to establish the epidemiological variables associated with episiotomies and their puerperal complications at Ahmadu bedlo university teaching hospital Zaria. The result findings suggest that the episiotomy rate was 35.6% of all vaginal deliveries . Episiotomies were significantly associated with primi gravida. The most common puerperal complications of episiotomies was asymmetry (32.9%), infection (23.7%), partial dehiscence (14.5%), hemorrhage (5.3%) and extension of the incision (1.3%). Nahid Arianpour 2009 (June) conducted a descriptive study to determine the bacterial species in postpartum infections at khanevadeh hospital. Postpartum infections among 6077 patients from 2003 till 2008 was studied in this study. In this study out of there different kinds of postpartum infections (i,e genital, breast and urinary tract ). Only genital infection is considered. Postpartum infections among 6077 patients from 2003 till 2008 was studied in this study. In result findings revealed that 7.59% (461 ) patients had postpartum infection, out of which 1.03%(63) patients were re hospitalized. The commonest infection was infection at the site of episiotomy. Kathelin Karla (2005) stated that the routine use of episiotomy harmful to the pelvic floor that creates greater extent of surgical incision and delayed perineal wound healing, compared with no episiotomy group. Continues improvement in quality of care after episiotomy could helps to control the infection and improves episiotomy wound healing. Nurse midwife plays on important role to promote postnatal care , to prevent complication of episiotomy wound earlier and hence force to bring a healthy mother. A well integrated postnatal care has an important role in assessing the transition to physical and psychological well being of the mother and baby . Episiotomy wound care is important for comfort and cleanliness and to prevent infection. Attention should be afforded to episiotomy wound care since it parallels any other wound. One of the suggested method is a regular antiseptic sitz bath. Nowadays using complementary therapies such as essential oils have been recognized in Obstetrics. Mechanism of action of each essential oils is related to its complex active substances .One of the main action of these oils are easy absorption through the skin. The molecule of essential oils are small enough to permeate through the skin barrier. The molecule will be absorbed easily into the skin within 20-40 minutes depending on the chemical nature of the oil. Lavender oil is one of the frequently prescribed essential oil due to its antibacterial, antifungal, sedative, anti depressant and healing properties. Research has been done to evaluate the effect of lavender oil sitz bath on episiotomy wound healing (Katayan Vakilan 2012). This randomized control trial was conducted on 120 primiparous women with singleton pregnancy who had received mediolateral episiotomy during spontaneous delivery. Redness in lavender oil group was significantly less than control group (p Fatemeh Sheikhan (Feb 2012) conducted a study on use of lavender oil essence in primiparous women in Iran. This clinical trial was involved 60 primiparous women. In result the REEDA score was significantly lower in experimental group (lavender group) 3 days after episiotomy (p=0.000). It is suggested that lavender oil essence may be preferably to the use of betadine for episiotomy wound care. Though there are many studies done in various methods of episiotomy healing, only a very few studies are conducted to assess the effectiveness of lavender oil sitz bath on episiotomy wound healing. So the researcher felt the need to evaluate the effectiveness of lavender oil sitz bath on episiotomy wound healing among postnatal mothers with episiotomy. Statement of the Problem A study to evaluate the effectiveness of lavender oil sitz bath on episiotomy wound healing among post natal mothers in government head quarters hospital at kanchipuram. Objectives To assess the level of episiotomy wound healing among postnatal mothers in experimental and control group. To evaluate the effectiveness of lavender oil sitz bath on episiotomy wound healing among postnatal mothers in experimental group. To determine the association between the level of episiotomy wound healing among postnatal mothers with their selected demographic and obstetric variables in experimental and control group. Hypothesis H1-There will be a significant difference between the mean post test level of episiotomy wound healing between experimental and control group. H2- There will be a significant association between the level of episiotomy wound healing with their selected demographic variables among postnatal mothers in experimental and control group. Operational Definition Effectiveness In this study effectiveness refers to the extend to which the lavender oil has produced desirable effect on episiotomy wound healing. Lavender Oil Sitz Bath It is a clean procedure in which the perineal area is immersed in 4 liter of warm water with a temperature of 100 105ËÅ ¡F added with 5 drops of lavender oil for 10-15 minutes thrice a day for 3 days is used. Episiotomy It refers to the surgical incision on the median or right/ left medio lateral aspect of the perineum during second stage of labour to enlarge the vaginal introitus, to facilitate easy and safe delivery. Postnatal Mothers It refers to mothers who have undergone normal vaginal delivery and instrumental delivery with episiotomy. Episiotomy Wound Healing It refers to absence of redness ,edema, ecchymosis , discharge and presence of well approximation of wound as measured by using REEDA scale. Assumption Presence of episiotomy wound may predispose to wound infection. Lavender oil aids in improving the healing of episiotomy wound. Delimitation The study is limited to the postnatal mothers who had normal vaginal and instrumental delivery with episiotomy. The data collection period is limited for six weeks. Projected Outcomes This study will help in assessment of episiotomy wound healing among postnatal mothers. This study will help to evaluate the effectiveness of lavender oil sitz bath on episiotomy wound healing among postnatal mothers. This study enables nurses to practice lavender oil sitz bath during postnatal period.

Early Onset Anorexia In recent years, it seems that the public has begun to pay more attention to eating disorders. This trend could be a consequence of the heightened nutrition and fitness craze that the 1990's has brought about, or possibly a result of more intense and conclusive research studies. Anorexia nervosa is a disorder that in the majority of cases will start when the patient is a teenager. The mean age at onset is figured to be about 17 years of age. Recently, child psychiatrists have begun to recognize increasing cases beginning in childhood (McCune & Walford, 1991). While severity of illness is usually associated with worse outcome, age of onset for anorexia can play a critical role in future outcome. Foundations of Management and Organisation

Is rationalization a desirable strategy for managing and organizing Junction Hotel in the current economic climate? Discuss your answer with reference to the topic of organisational change. Rationalization is an organisational concept that aims to increase a firm’s productivity, as it grows in size, through a reorganisation of its current system of operations. Throughout this essay I will be referring to the Junction Hotel case study along with other articles and critiques of the rationalization theory to determine how suitable rationalisation is as a solution for the problems facing Junction Hotel within the current economic climate.Throughout the 19th century Frederick Taylor, a mechanical engineer, was one of the pioneers of the organisational management approach to business. He was renowned for his theory of scientific management, which focused primarily on increasing the physical efficiency of the individual worker. â€Å"The principal object of management should be to s ecure the maximum prosperity for the employer, coupled with the maximum prosperity for each employee† (Fredrick Taylor, 1911, pg. 09).One of Taylor’s most famous studies involved designing shovels that could load the most efficient amount of material (21 1/2lbs) consecutively to save workers time and, in turn, increase productivity. Having a clear and structured command over workers allowed Taylor to experiment with efficiency, which was something he saw as paramount to having a successful workforce of employees. Relating this specifically to the Junction Hotel case study, it is apparent that there are some clear organisational problems that could be resolved using a more rational work design, such as the methods introduced by Taylor.Primarily there is a lack of any concise structure between the task expectations of the employees. The job roles of the General Manager, Deputy Manager, Head Chef and Company Accountant all contradict one another making it further unclear t o the rest of the workforce who they should report to as their boss. A rational organisational solution to this problem would be to introduce a more modern, bureaucratic style of management. â€Å"Though earlier societies had organisational structures, they were not nearly as effective as the bureaucracy† (George Ritzer, 1996, pg.  09).This can be implemented through the use of computer analysis of such things as employee timetables, busy periods, and popular items among customers. Specifically, organisational charts, which provide a clear formation of different job roles that are allocated in a hierarchy of either horizontal or vertical structure, would work effectively in restoring order and control. A hierarchy is most effective in assigning control to different regions of management in order to maintain a more specific level of control over a growing workforce.For Junction Hotel, this would be most appropriate for the separation of the different working areas, which, at the moment, are all under the control of the Deputy Manager. Instead, each area should be operated by group of employees that specialise in that service. From here, each section of workers can then have their own specific manager to report to for daily duties. This way the general manger can take more of an active role within the organisation relieving some of the pressure off of the Deputy Manager and allowing the Head Chef control over his own workforce.This would provide specific segmentation to each individual work force, allowing for a more direct focus from each manager. This would be the most appropriate way of applying a rational, bureaucratic work design to Junction hotel without having to sacrifice any quality of service. However, if Junction Hotel want to remain a competitive company within the hotel industry, some of the personal relationships between the cleaning and reception staff and the general manager may have to be limited in order to increase efficiency during t he working day.Taylor made it clear that he never saw the benefit of a social relationship between workers and managers. Eventually, he became so focused on increasing productivity that he began to see his employees as just another ‘cog in the machine’ of the production process. As time went on, employees would try and use this to their advantage by demonstrating their ‘asymmetric knowledge’. After years of practising their trade the workers would be able to determine the amount of time it would take to complete a specific task before they began it.With Taylor’s approach to his employees being so cold-hearted, it was often the case that the employees would exaggerate the time they needed to complete a task so as to insure they weren’t being over worked. This is an example of some of the social problems that Taylor had to face due to having such a strong rational approach to work. As a result, Taylor quickly began gaining criticism. Max Weber, a German sociologist, was one of Taylor’s biggest critics. Weber summarised Taylor’s scientific management theory as an ‘iron cage’ environment leaving workers trapped in working conditions that left them feeling dehumanized.As a counter belief, Weber explained that promotion by merit with career opportunities for employees was a superior option to the traditional hierarchy of power. However, Weber was aware this would not always be attainable in a realistic world. â€Å"Weber knew the bureaucracy he was designing was an ideal type and that it could not always be perfect† (French & Rayner, 2011, pg315-316). Weber developed the belief that there is no one perfect method of managing an organisation. Organisational change occurs when a business has a need to alter it’s current system of operations.Social, political, technological and economical factors are the four main external aspects that can influence change within an organisation. â€Å"No t only are there different types of change, which manifest themselves in different organisations, change also appears at different levels of an organisation† (Barbara Senior, 2001, pg. 57). Specifically, organisational change relates to the larger changes within a company, rather than just small adjustments to products or branding.â€Å"Organisational change refers to organisation-wide change rather than to small changes such as adding a new role or making minor modifications to a process. † (French & Rayner, 2011, pg. 574). For example, advancements in the technology used by a company would dramatically speed up the production process forcing them to alter the size of both their workforce and production outlets. Henry Ford, of Ford Motor Industries, was one of the most famous examples of an organisation implementing a change within their production process  through the Taylorism ideology.â€Å"Taylor and scientific management allied to Fordism† (Senior & Flemi ng, 2006, pg. 8). Instead of having a specific number of workers collectively working on one motor vehicle, Ford instead realised that he could save time and increase productivity by assigning each individual worker to a particular station on his assembly line. Therefore, Ford could just move the car around the assembly line where it would stop at each station to have a new part added to it.This enabled Ford to expand and increase his workforce whilst still increasing the amount of automobiles he was producing. â€Å"Only by doubling wages to his famous ‘5-dollar-a-day’ was he able to stabilize the work situation and persuade workers to accept the new technology† (Morgan G, 2006, pg. 25). This organisational change would be an example of a naive approach to change, which sees organizational change as a simple solid set of building blocks that are controlled from the top down, with a limited view of only the techno-structural side of the organisation.With one of t he main purposes of a business being to constantly strive to improve output and lower costs in order to increase revenue, the Taylorism and Fordism approaches became popular around the world and production efficiency was now of the utmost importance. However, this high level of efficiency came at a price for the employees. Many of Taylor’s and Ford’s workers found it difficult to maintain any humanistic grasp on their machinelike work due to its repetitive and boring nature.This led to a decrease in efficiency as workers became fatigued and after a while could no longer keep up with the rate of production. Charlie Chaplin outlines this perfectly in his video Modern Times (TheCharlesChaplin, 1936, Modern Times) whereby the worker is dragged along the conveyor belt assembly line because he is incapable of keeping up with the pace. In the Junction Hotel case study a similar situation is outlined with the front of house staff not being able to keep up with the orders given by the kitchen staff, leading to conflict between employees.Ultimately, this will lead to problems in the satisfaction levels of the customers, which can cause huge reputation problems for Junction Hotel, a company who pride themselves on having a high level of traditional customer service. One of the most effective ways of tackling employee confrontation is through the use of team building exercises. Team building exercises are effective as they encourage employees to collaborate with one another to achieve particular goals without having the stress element of actual work.Being new to the team building approach, the waiting and kitchen staff at Junction Hotel would benefit most from a more relaxed outdoor operation, such as raft building. â€Å"For a group that has never done team building before, outdoor experiences can be an exciting way to begin† (French & Rayner, 2011, pg. 410). As a result, when both sets of employees go back to work there will be a greater understandi ng between each member of staff creating a new atmosphere as a group identity. This social aspect of employees during working hours is a factor that was often overlooked by traditional methods of management, such as Taylorism.As a result, this 21st century social method of management is now considered a more popular modern alternative than the traditional Taylorism perspective. In the 1920s, Elton Mayo, an Australian psychologist, stumbled upon the importance of group dynamics whilst conducting a study investigating the levels of productivity in employees. His experiments, on 29,000 workers at the Hawthorne factory, examined the effect light had on productivity. His objectives were to discover the optimum level of lighting needed to gain maximum efficiency from workers.However, Mayo found that the level of lighting had no direct effect on production levels but instead the human cooperative systems that the workers were a part of had a huge effect on increasing worker efficiency. Wor kers being able to establish themselves within an occupational community introduced the concept of ‘group norms’ whereby workers had their own set of regulations to abide by that were considered more socially acceptable. â€Å"Members derived valued identities or self-images, directly from their occupational roles. † (Van Maanan & J & Barley, S, 1984, pg.  298).Expressions and labels such as; ‘rate buster’ and ‘the chisler’ became nicknames that were used to define workers as either an over-worker or under-achiever. The power of an informal organisation, where man is more than just a machine but as a social being was one of Mayo’s greatest findings. This became known as the Hawthorne Effect. Conversely, a bureaucratic style of management is most appropriately suited to those organisations that are willing to sacrifice quality in return for an increase in quantity of revenue.For example, Travelodge, a competing company within the hotel industry, make it clear to their customers that the service they offer is simple but covers all the necessary essentials needed for a comfortable stay. â€Å"If you’ve ever stayed at a Travelodge Hotel, you might have noticed they don’t have shampoo in the bathroom. † (Davis, Evan, 2009) This is known as value engineering and it is a smart way for larger companies to cut production costs whilst still keeping up a respectable level of quality.Higher-end hotels tend to ignore this strategy as it can harshly diminish their unique selling point of providing the best quality goods and services. This would be particularly true for Junction Hotel. Throughout the case study it is clearly outlined that Junction Hotel are a company who pride themselves on having a traditional approach to customer service. This would specifically involve having a very high focus on self-presentation, customer relations, and particularly individual customer requirements.Rationalisati on revolves more around a very routine and strategic attitude whereby all customers are greeted with the same standardised mannerisms and production is aimed at being a quick process, which sacrifices quality for speed. This is the first indication from the case material that suggests that rationalisation might not be the most appropriate method of management for a company that markets itself as ‘an oasis of calm in the city’ as Junction Hotel does. McDonald’s is a perfect example of how quality over quantity has been sacrificed to maximize output.A quick service where customers can walk in and point to a number on the menu as they order means workers can speed up the transaction process of payment for food allowing them to switch to new customers swiftly. â€Å"The manager ensures that all employees are allocated to those jobs in which they perform most efficiently, known as ‘aces in their places†. (Hill, Terry, (2005) McDonald's Corporation). This type of McDonaldization means there is no room for flexibility making it a real problem if customers request to manipulate the menu.This slows down the  process from kitchen staff to front of house staff, if one individual burger is needed to be adapted this will take precious time out of an employee’s schedule, which will have a domino effect on the time taken to serve other customers. Ultimately, the efficiency of the entire operation will have slowed dramatically, especially if this happens on more than one occasion throughout the working day. As Weber explained, dehumanization and lack of motivation are due side effects as a result of having such strict time schedules and regulations to keep to.â€Å"Nevertheless, organisations continue to strive for maximization in the hope that they will at least increase efficiency† (Ritzer, G, 2008, The McDonaldisation of Society). Junction Hotel, on the other hand, has a slightly different demographic to that of the market t hat McDonalds are focusing on. With the emphasis on high-end, premium quality goods and services Junction Hotel are attracting an older age range of those customer who can afford to spend extra on hotel costs. This will most likely be older people enjoying their retirement along with businessmen and women who have their expenses paid for.The reason this is important to identify is because of the current economic position of the UK. Having just emerged out of a recession, consumer spending within in the UK is still continuing at slow pace. This means, that for the more expensive and luxurious organisations it has become just as hard to keep current customers as it has been attracting any new ones. This is the second indication that suggests a complete rationalisation for Junction Hotel would not have much of a positive impact considering it would only increase the similarities they have with already well established rival companies such as Travelodge.In an economy that is still recov ering from a recent recession due to a ‘run on the banks’ consumer confidence is still very low which means less people are going to be willing to experiment with new organisations, especially the more expensive ones. However, some of the most recent economic reports have identified a rise in gross domestic produce, which is a positive sign for Junction Hotel as it means more consumers are starting to get comfortable with spending a little extra money.â€Å"The Office for National Statistics said its first estimate for gross domestic product (GDP) showed the economy grew 0. 3% during the first quarter of 2013. † (Hugh Pym, 2013, UK economy avoids triple dip recession). Although the increase is not a significant one this will benefit Junction Hotel in the long run as it shows signs of a slow but steady growth which is a good climate to begin setting up a new organization in. â€Å"Economists say the news should give a small psychological boost to consumers and bu sinesses†. (Hugh Pym, 2013, UK economy avoids triple dip recession).Keeping with the premium quality approach should provide a competitive edge in the favour of Junction hotel. This type of organizational change is a strategic intervention to provide Junction Hotel with a competitive advantage over their rivals in the hotel industry by aiming their services at a different demographic who will, in the current economic climate, be more likely to afford hotel accommodation. In conclusion to this essay, Junction Hotel will need to introduce a more rational work design to their organisation in order to maximize efficiency and start making a respectable profit.It is vital however that the rational approach is put into action in the most applicable places of the organisation as otherwise it could cause a detrimental effect to production the rates, as was evident with some areas of the McDonaldization. Firstly, a bureaucratic design should be implemented through the use of organisatio nal charts that clearly layout the different tiers of management throughout the entire organisation in a hierarchal fashion.This should enable each section of the work force to have a structured view of where they stand within the organisation and know exactly who to report to. Secondly, team-building exercises must be put into place at least quarterly throughout the year to dispose of any unwanted negative energy and allow workers to voice any concerns they may have with their superiors, especially kitchen and waiting staff. A formal relationship must be of the utmost importance in front of customers between all of the employees.Although, where customer service is not of a high importance, such as in the kitchen, a more relaxed attitude should be taken if it means the social side of individual worker will promote a higher level of efficiency. Overall, it is clear that with the right leadership and a positive attitude towards organisational change from the workers, rationalisation i s strategy that could have significant positive implications for Junction hotel.

This paper will describe pain as it relates to nursing, medicine, and physical therapy. All of the information gathered is analyzed in reference to how pain relates to hospital stay, rehabilitation, and prevention. I will ultimately give an overall picture of the importance of pain and how it relates to nursing as well as other disciplines. Introduction This paper is a concept analysis of what is known as the fifth vital sign PAIN.As stated by Hsiao, Wu, & Chen (2013), Nursing staff are the major group of healthcare professionals who perform crucial functions in delivering nursing care to inpatients. I will attempt to describe pain as it relates to nursing, medicine, and physical therapy. When selecting a concept, I began by thinking what is an important factor in the nursing field and could be analyzed in more detail. Pain is a factor that is thought of d ifferently by many. It is a very subjective factor and that applies to the patients, physicians, and nurses.Yes, it is the patient that feels the pain but it is the physician who writes the orders and the nurse who transcribes it. I then figured that this area of nursing practice would be a great concept to analyze. Pain is a feeling triggered in the nervous system. Pain may be sharp or dull. It may come and go, or it may be constant. You may feel pain in one area of your body, such as your back, abdomen or chest or you may feel pain all over, such as when your muscles ache from the flu. Pain can be helpful in diagnosing a problem.Without pain, you might seriously hurt yourself without knowing it, or you might not realize you have a medical problem that needs treatment. Once you take care of the problem, pain usually goes away. However, sometimes pain goes on for weeks, months or even years. This is called chronic pain. Sometimes chronic pain is due to an ongoing cause, such as canc er or arthritis (â€Å"Pain†). In the nursing field as well as any other areas of healthcare, pain is a vital factor when dealing with the delivery of care. It is the responsibility of he physician to ensure relief from pain by writing the orders, it is the responsibility of the nurse to carry out the orders and give the pain medication, and in the area of physical therapy it is their responsibility to ensure that the patient is medicated to retrieve the best results while in therapy. Purpose and Method The purposes are to define what pain is considered to be in relationship to the patient, nurse, physician, and physical therapist. It is also, to find out the subjectiveness of the fifth vital sign and to explore pain as it relates to hospitalization.Walker & Avant (2005) guided this concept analysis study. Pain in Nursing Almost 35 million patients were discharged from U. S. hospitals in 2004; of these patients, 46 percent had a surgical procedure and 16 percent had one or mo re diagnostic procedures. Pain is common, and expected, after surgery. Recent data suggest 80 percent of patients experience pain postoperatively with between 11 and 20 percent experiencing severe pain, (Wells, Pasero, & Mcraffery, 2008).There are many contributing factors as pain relates to nursing. Pain is the main factor that contributes to a person deciding to seek medical attention. It is very important to include this as a part of an assessment because it can affect so many other things. It can cause a blood pressure to be elevated, it can increase a blood glucose, and can cause an overall disturbance in the patients’ hospital stay or ADL’s. In nursing our aim is to provide comfort and surrccome to the patients’ needs as a part of their overall care plan.As stated by Wells, Pasero, & Mcraffery, 2008, inadequately managed pain can lead to adverse physical and psychological patient outcomes for individual patients and their families. Continuous, unrelieved p ain activates the pituitary-adrenal axis, which can suppress the immune system and result in postsurgical infection and poor wound healing. Pain in Medicine Medical professionals have a big impact on pain as it relates to healthcare. Physicians have to generate a care plan that will ultimately be in the favor of the patient as far as relieving the pain in the most appropriate way.This is achieved by not altering consciousness, normal ADL’s, and in the correct combination with other medications. The board strongly urges physicians and surgeons to view effective pain management as a high priority in all patients, including children, the elderly, and patients who are terminally ill. Pain should be assessed and treated promptly, effectively and for as long as pain persists. The medical management of pain should be based on up-to-date knowledge about pain, pain assessment and pain treatment (â€Å"Guidelines for prescribing,† 2007).Nociception is the term used to describe t he neural processes by which a noxious substance or a tissue damaging event such as surgical incision is perceived as pain (Fig 1). This is described in four stages, transduction, transmission, perception and modulation. Nociception involves a complex interaction between the peripheral nervous system (PNS) and central nervous system (CNS) as well as an evaluation of patients’ pre and post-operative psychological and environmental influences (Wood, 2010). Pain in Physical TherapyDue to factors such as surgery, stroke, or basic deconditioning there is a need for rehabilitation. Physical Therapist play an essential role in the patients’ road to recovery. A patient-centered rehabilitative approach that emphasizes restoration of normal movement and function incorporates physical therapy as a vital component of the collaborative approach required for effective pain management (â€Å"Physical therapy for,† 1998). Overall therapy is lessened with pain. Patients move slo wer and are less involved with therapy if pain is present.Therapy is put into place to get those muscles moving and to later decrease pain as it relates to contributing factors. These factors may include surgeries (hip or knee replacements), amputations (AKA, BKA, Metatarsal, and digital), and weakness (from CVA, CABG, and decrease movements). Patients with pain perceive an equivalent level of exertion at a significantly lower level of performance, a finding accounted for by both central (cardiorespiratory) and peripheral (muscle strength and recruitment) factors.Inactivity also deprives bones, joint cartilage, and connective tissue of the mechanical stress necessary to maintain tensile and compressive strength and elasticity. Evidence is building that motor control and proprioceptive efficiency are altered, balance is compromised, and reaction times are slower in persons who are unfit or have pain (â€Å"Physical therapy for†, 1998). Essential/Critical AttributesAccording to Cheng, Foster & Huang, (2003) the critical attributes of pain include: (a) unpleasant and distressful experiences originating from physical sensation and having both positive and negative meanings for an individual; (b) an individual human experience; (c) a state of feeling in both sensation and emotion (verbal), and behavioral components; (d) physical and psychological responses to the stimulus; (e) function of pain, including protective and warning signs; (f) pain responses are learned and influenced by personality, environment, emotions, social and culture. Model Case Mrs.K is a 37 year old female with recent diagnosis of breast cancer. She has recently undergone a mastectomy and is now in the hospital for recovery. Upon the nurses’ hourly rounds she discovered Mrs. K crying, when asked what the nurse could do to help she stated that â€Å"I am in pain and can’t believe that I only have one breast now, I’m ugly and would have never thought it would hurt thi s bad†. The nurse consoled her and brought pain meds. When returning the patient thanked the nurse for the talk and she stated that the pain had eased and she always thought she would have breast cancer due to both her mom and grandmother having it.Related Case Mrs. T is a 35 year old female with breast soreness and discharge. She was found to only have fibroid tissue and nothing metastatic in her breast, but due to family history she elected to have bilateral mastectomies. She was ordered a PCA pump due to hourly pain calls and when the nurse entered the room she was crying c/o continuous pain. She states â€Å"I can’t take this pain and I hate I did this†. The nurse took time to talk with the patient and informed the physician of her crying and got new orders.During the nurses hourly rounds she found her sitting up on the phone and she explained to the nurse that she feels better and glad that she had the surgery because she did not want to go through what her mother and grandmother went through with breast cancer. Contrary Case Mr. M is a 45 year old that had a motor vehicle accident when he was 20 and is now going from hospital to hospital to try to get IV pain medication. He complains of back pain but x-ray results does not show any back issues but due to out of control behavior he is admitted and given pain medication. He refuses all by mouth pain meds and demands IV meds.He does not show any signs of pain when assessed and all staff believes him to be addicted to pain medication. A psych consult is ordered. This patient is not in true pain but has psychiatric issues. Antecedents and Consequences For this particular concept the antecedents and consequences would be all interrelated due to the fact that in nursing, medicine, and physical therapy the same factors apply for pain. Environmental, personal and cultural values act as antecedents related to the concept of pain. The consequences of pain are related to pain reaction and the ind ividual's own interpretation of the meaning of pain.Pain reaction has a more physical and biological focus; coping with pain refers to the perception of pain and is psychologically focused (Cheng, Foster ; Huang, 2003). When thinking of environmental, personal, and cultural values you begin to think of things such as the patients comorbidities, what the patient perceives as pain, and in their culture how is pain thought of. In many cultures it may be thought of that you must tolerate pain as long as you can while in other cultures their pain threshold is low so any uncomfortable feeling renders a pain medication.Consequences ultimately come from the patient themselves, what is perceived as pain and their history of pain. For example, a patient that has had a laminectomy may think because the pain was horrific the first time that it will be the same the next time, expecting the same relief factors even if the pain is not the same. Empirical Referents Empirical Referents are stated as what it used to measure the concept. Pain is very subjective and they have yet to invent an objective test that will give you the accuracy of some ones pain. They are measured by a scaling system.Most scales make pain measurable, and can tell providers whether your pain is mild, moderate or severe. They can also set baselines and trends for your pain, making it easier to find appropriate treatments. This includes the Numerical Rating scale asking for a pain rating on a scale of 1-10, the Wong-Baker scale which will include facial expressions to emphasize pain, or the Verbal-rating scale and in this scale the patient must complete a questionnaire to describe their pain in more detail (Jacques, 2010). Implications in Nursing In nursing pain is a major factor in treating the patient as it relates to the overall hospitalization.How are we able to get the blood pressure down if the patient is in constant pain? or how can I discharge a patient when they are in a lot of pain? Implications in nursing goes for all aspects of nursing from skin integrity to normal ADL’s. A patient may not turn as they should because they are in the fetal position due to pain which could cause skin issues or pain will cause them not to take a bath as usual. Pain can even cause depression as it decreases normal functioning. This concept is very important in the nursing field and should be treated and assessed appropriately.Conclusion In conclusion I chose to analyze pain as it relates to nursing, medicine, and physical therapy. It is a major factor in that it is related to all aspects of care that is received by these disciplines. Pain is the main contributing factor that causes someone to seek medical attention. Without pain we could have serious issues being that we would not know when were sick, burned, or injured. Pain is very important in healthcare and applies to nursing, medicine, and physical therapy.
